Using the form below, enter your details and create your profiles. You will be able to upload a copy of your plan and budget there. We will contact you to finalize your sign-up process and service agreement.

If you have difficulty with fill up and submit the form below, please send an email to account@pmndis.com.au with your details including your – name, NDIS number, and a copy of your current plan. We will contact you via phone call or email within 2 business days to forward you a service agreement and complete your sign up.

If you are transferring your plan management from another plan manager please first contact the NDIA on 1800 800 110 to advise them, and request them to cancel the existing service booking for your old provider.

Please contact our team via email account@pmndis.com.au with a copy of your new plan attached. We will update your account with your new plan.
